​About the Horizon Home (ICF) at Forest Hills

Tucked into the heart of Lynchburg, Horizon Home at Forest Hills is an Intermediate Care Facility (ICFs) with ten residents. Originally built in 1993, this home has been updated to create a therapeutic living environment for all residents. As one of seven ICFs located throughout Central Virginia, Horizon Home at Forest Hills provides each resident the opportunity to be involved in active treatment to enhance their functional skills while creating an environment where they thrive in everyday life.

Each resident of Horizon Home at Forest Hills has an interdisciplinary care plan that is developed for their specific needs in order for them to fully thrive and immerse themselves in the community they live. The care one receives in the Intermediate Care Facility promotes health, self-worth, and family/ community involvement in the least restrictive setting.

Horizon's ICFs also support individuals with a variety of medical and behavioral needs. Care is provided by qualified staff, 24 hours a day, seven days a week. Horizon has been providing these services to the communities around Central Virginia for more than 30 years.

Criteria for Admission: The client must be over 18 years of age and have a primary diagnosis of intellectual disability. Adults that have a dual diagnosis of mental illness and intellectual disability can also be served in our programs. All prospective residents are assessed by the Department of Behavioral Health and Developmental Services to meet the criteria for an ICF program. Horizon's Admissions team will also conduct a review to determine eligibility.
